As a parent, you have the right to guide your child’s upbringing and education, including their health care, religious formation, and exposure to sensitive topics at school. Our free Parental Rights Opt-Out Form helps you tell your child’s public or charter school, clearly and in writing, which programs, services, surveys, and instruction you do not consent to.

The form covers four areas: health and medical services, surveys and personal inquiries, sex and gender content, and wellness programs rooted in religious practices. It also includes supporting references to federal and state constitutional law, including the U.S. Supreme Court’s 2025 decision in Mahmoud v. Taylor. Check the items that apply, sign it, and submit it to your school. Be sure to keep a signed copy for your records.
